Qualified Health Center (FQHC) and a Patient-Centered Primary Care Home (PCPCH)

We are looking for a Community Alignment Specialist who is ready to lead resource
coordination, grow connections with local community-based organizations, and make a
lasting impact on our public health mission. The Community Alignment Specialist
executes core Family Connects Oregon (FCO) deliverables and serves as Winding
Waters' pediatric and family Community Health Worker.
The Impact You’ll Make
● Be the Coordination Lead: Partner with the Family Connects Oregon (FCO)
nurse to identify family needs during home visits and connect them with
resources. You will own the development of the FCO Wallowa County Provider
Directory, a comprehensive registry of health and social services for local
families.
● Drive Community Alignment: Cultivate and strengthen partnerships with local
healthcare providers, social service organizations, and community groups to
align local resources. You will also spearhead Wallowa County's FCO
communications plan and support the Program Administrator in developing and
running the Community Advisory Board (CAB).
● Navigate Patient Care & Social Needs: As a Community Alignment Specialist,
you will provide direct, one-on-one navigation support to help patients schedule
appointments, understand care plans, and overcome barriers to care. You will
screen patients for Health-Related Social Needs (HRSN)—such as food,
housing, and transportation—and act as a dedicated advocate to connect them
with vital community resources and social services.

● Advocate for Your Patients: Serve as a critical link to our Medical-Legal
Partnership (MLP). You will identify patients with legal needs impacting their
health—such as public benefits or family stability—and act as a liaison between
the patient, the clinical care team, and legal professionals.
● Deliver Health Education: Additionally, you will lead local outreach initiatives
and deliver culturally appropriate health education to promote clinic services and
public health programs.
What You’ll Bring to the Team
● A Certified Expert: You possess a strong understanding of Community Health
Worker (CHW) competencies and hold (or are committed to obtaining within 6
months of hire) an Oregon Traditional Health Worker certification.
● A Master Communicator: Exceptional oral communication skills, utilizing
motivational interviewing to build instant trust and rapport with families from
highly diverse backgrounds.
● A Tech-Savvy Advocate: Highly proficient in Electronic Health Records (EHR)
systems, Microsoft Word, Microsoft Excel, and Google Docs. You understand
how to leverage internet resources safely while strictly maintaining HIPAA
compliance and data privacy.
● Physical Resilience: You possess the stamina to navigate long hours across
both clinical spaces and mobile community outreach environments, including the
manual dexterity to type/write and the ability to lift up to 35 pounds.
Preferred Experience & Attributes:
● Lived Experience: Personal experience successfully navigating maternal health,
childbirth, lactation, and early childhood support systems.
● Regional Knowledge: Firsthand familiarity with the social services, healthcare
networks, and community resources available to families in Wallowa County or
the surrounding region.
● Peer Support: A strong desire or background in using your personal journey to
mentor, advocate for, and empower local mothers.
Success in this Role Looks Like:
● High Patient Engagement: Maximizing the total number of local families
successfully connected to community resources and nutrition benefits.
● Seamless Operations: Meeting established community outreach goals while
ensuring all patient interactions and progress notes are documented in a
comprehensive and timely fashion in the Electronic Health Record.
● Collaborative Spirit: Showing active, reliable participation in all departmental
